Prior to making an investment, the investor will be required to submit the following documents:

• Specimen Signature Cards
• Investor Risk Profiling Questionnaire
• Investment Declaration, if applicable
• Risk Disclosure Statement
• Participating Trust Agreement
• Valid IDs of the investor

Additional documents for Corporate Accounts:

• Copy of the SEC Registration
• Copy of Articles of Incorporation and By-Laws
• Notarized Secretary’s Certificate authorizing the corporation to open an account with EastWest Bank – Trust Division and appointing the designated officers authorized to sign for and in behalf of the corporation
• Trust Account Opening Form for the signatories
